Abstract

I felt as though an examination beyond technological failure would be necessary to reveal the overarching practices that surround and constitute aviation failures. To illustrate this argument, I researched one well-known aviation accident: the fateful 1988 fuselage failure of Aloha Airlines Flight 243. Revisiting this accident with modern understandings of causal structures and processes helped unmask the key organizational elements precipitating this disaster.
